FIVE (there's a new one!) steps for chaperones to register for QYLC 2012

1. Email Angie Martin (angie_martin@fa.org)  letting her know how many students and faculty you are bringing.  

2. Fill out the Online Registration Form by Friday, January 6th.  

        You will need to know the following information for each student and chaperone in order to register:
- name
- dietary restrictions
- activity preference (bowling or ice skating)

3. Mail in your registration fee of $75 per person (students and chaperones) by Friday, January 6th.

Make checks payable to Friends Academy with QYLC in the ‘memo’ area and mail to:

4. Download and have each student's parents fill out a Medical Consent Form.  

Bring two copies of each form with you to the conference, one for you to carry and one for the conference organizers. 

5. NEW!!! Fax us a certificate of insurance naming Friends Academy as an additional insured

I apologize for this additional step, but I was just informed that we need all participating schools to fax us a copy of their school's certificate of insurance naming Friends Academy as an additional insured.  Our business office says that this is standard procedure and shouldn't be a problem.  You just need to pass along the following information to your business office and ask that your insurance company fax us the certificate of insurance naming Friends Academy as an additional insured.
